Case Management

We provide comprehensive, client-centered support to empower families in overcoming challenges and accessing essential services for enhanced well-being.

Our team collaborates with over 50 community partners and service providers to ensure families receive the assistance they need, from housing and healthcare to financial and legal services. We have dedicated more than 180 hours to crisis management, assisting over 130 families navigating significant hardships, with numbers growing year to year.

Please note: Only currently active Heart of Los Angeles families are eligible for case management services.
